Get your sleeve ready, and let’s do this!
How to Use a Penis Extender: The Basics in 60 Seconds
Short on time? Here are the basic instructions on how to use any cock sleeve safely and effectively.
- Lube up: Apply a bit of water-based lube to the head of your penis for comfort. Add some to your shaft as well. Don’t put too much, or the sleeve will be too slippery and won’t hold tight to your D.
- Slide the sleeve on: Carefully insert your penis into the penis sleeve. Slide the sleeve down to the base of your shaft. The fit should feel a bit tight but not uncomfortable or painful.
- Secure the sleeve: Most cock sleeves hold tight to your dong via a vacuum seal or a scrotum loop. In the case of the former, squeeze the sleeve to remove the air and suction it to your dick. For the latter, place your scrotum through the loop to hold the sleeve in place.
- Lube up—again: Before penetrating your partner, you need to make sure your sleeve is nice and slippery. I recommend a water-based lube like Sliquid H2O. A lack of lube can hurt your partner or damage your sleeve.
- Go slow: The dimensions of your dick have increased at this point, so you want to go slow to ensure your partner can handle your new size comfortably. Also, many sleeves are made with delicate skin-like materials that can tear if you’re not gentle.
- Stick with standard positions: Stick with positions like missionary, doggy, and others that won’t pull or bend the sleeve. Some positions, like the spooning position, may cause the sleeve to slip off. Of course, some sleeves are more durable than others, so feel free to push the limits if you want!
- Finish strong: You can finish inside the penis sleeve if you want. Just be careful, as your man juice may leak out the base of the sleeve. Your sleeve should hold tight even after you’ve finished, so continue going to town if you’re not ready to quit!
- Clean up: Cleaning your cock sleeve is really important. A dirty sleeve can give you or your partner an STI and/or damage the toy. Wash the sleeve right after using it, especially if you finished inside. You can find washing instructions on the toy’s packaging (I’ll also talk more about cleaning later in this article)
How to Use Each Type of Penis Extender
There are a number of different types of penis sleeves out there, and each one works a bit differently.
To learn how to use a penis sleeve properly, you need to understand the different methods of securing a sleeve to your dick. Most sleeves use one of the following methods:
👉 Vacuum seal: This type of sleeve holds tight to your dick with the power of suction. For an example, check out the Fantasy X-Tensions Mega 2” Penis Extension Sleeve.
👉 Scrotum loop: This sleeve has a strap around its base that wraps around your balls. See the Performance Realistic Cock Xtender.
👉 Scrotum hole: This sleeve has a hole in its base that you put your balls through. Check out the Renegade Manaconda Clear Cock Extender Sleeve.
Below, I’ll briefly explain how to use each sleeve.
Note: I’m not going to repeat all the basic instructions from above for each type of sleeve. I’ll just note the differences that make using each sleeve unique.
How to Use a Vacuum-Seal Penis Extender
If your penis sleeve has a vacuum seal, you don’t have to worry about holes, loops, or straps that some people find uncomfortable against their scrotum. Here’s how to use this penis extender.
👉 Lube lightly: Vacuum seal penis extenders need to maintain an airtight lock on your shaft, so don’t add too much lube. If the sleeve is too slippery, it won’t stay on. You should apply some lube to the head of your penis, though.
👉 Apply the sleeve: These sleeves are the easiest to put on because there aren’t any straps in the way. Just slide it onto your shaft until it reaches the base.
👉 Squeeze the air out: Give the sleeve a gentle squeeze to force the air out of the bottom of the toy. This will cause the sleeve to lock onto your shaft. The seal should hold tight even if you become flaccid.
👉 Plow gently: Remember that twisting or bending the sleeve may break the airtight seal and cause the toy to come loose. This isn’t a big deal, but it might ruin the mood if you have to pause to re-fit your sheath.
How to Use a Penis Extender with a Scrotum Loop
Penis extenders with scrotum loops are popular because they tend to hold tight even if you get a bit rowdier in the sack. Here’s how to use one.
👉 Lube the loop: Apply plenty of water-based lube to your balls and the penis sleeve’s loop before putting the sleeve on. The loop should fit tight to keep the sleeve securely on your dick, but that means you’ll need to lube up well to avoid any discomfort when wrapping it around your scrotum.
👉 Slide the sleeve on: Slide the sleeve onto your erect penis before putting your balls into the strap. When you put the sleeve on, the empty loop should hang below your shaft.
👉 Gently put your balls through the loop: Push your balls through the lubricated loop one at a time to avoid any discomfort. Make sure to put your scrotum fully through the loop. It shouldn’t be putting any pressure on your testicles.
👉 Go to town: Now, your penis extender is snugly in place. Thanks to the security of the strap, you can be a bit more adventurous than you would with a vacuum seal penis sleeve. So go to town, and have fun!
How to Use a Penis Extender with a Scrotum Hole
Penis extenders with scrotum holes offer even more security than sleeves with loops, but you may have a bit less freedom of movement.
These are not the most popular sleeves, especially for guys whose scrotum retracts a lot when they’re having sex. However, they can be a good option if other sleeves aren’t secure enough.
👉 Lube liberally: Apply plenty of lube to the area of the penis sleeve around the hole. Also, lube up your scrotum and the base of your shaft. This will make it easier to fit your balls in the hole and prevent discomfort during sex.
👉 Slide the sleeve on: Slide the sleeve onto your penis until you can’t push it further down the shaft. The hole should be facing down. Your penis will only be partially inside the sleeve at this point, and that’s ok. You’ll be able to slide it on completely once your scrotum is in the hole.
👉 Gently put your balls through the hole: Stretch the scrotum hole and gently maneuver your balls into it, one by one. Once they’re inside, you will be able to slide the cock sleeve the rest of the way onto your penis.
Which Type of Penis Extender Has the Best Fit?
This really depends on the person. All penises and scrotums are unique, so different people prefer different types of penis sleeves.
Some people love vacuum seal penis sleeves because they tend to be the most comfortable.
With no strap and no hole, your balls are free and unrestricted. This feels more natural and removes the possibility of any uncomfortable rubbing.
However, for others, these sleeves are just not secure enough. Or, they want to use them in positions that may break the airtight seal, making the sleeve harder to keep on.
If you want a natural look and feel, a vacuum seal sleeve is best. But you’ll need to be a bit gentler during sex.
Sleeves with scrotum straps are more secure, so you’ll be able to do more in bed and worry less about the sleeve coming off.
However, these sleeves feel less natural, and some people don’t like the sensation of a strap around their scrotum.
Still, these sleeves are quite comfortable, and if you’ve worn a cock ring, the feeling of a strap around your scrote won’t be anything new.
Penis sleeves with scrotum holes are quite secure, and for some, they feel the most natural because they hold tight and let your balls hang.
For others, they can be quite restrictive on the scrotum and cause some uncomfortable rubbing.
Which Penis Extender Should You Try?
I’d recommend starting with a vacuum seal penis extender like the Fantasy X-Tensions 1” Penis Extender.
Adding just 1” of extra length, it’ll be easy to keep on and more manageable for your partner. The realistic look and feel of these sleeves make them great gateways into penis extender play.
If you want to try more cock sleeves or get a bit more adventurous between the sheets, you can opt for a more secure sleeve with a strap, such as the Performance 11.5” Cock Sheath.
This bad boy adds 2.5”, and it has a strap and suction power for a super secure fit.

Penis Extender Safety: What You Need to Know
You might be wondering if penis sleeves are safe, and the answer is yes. Penis sleeves are among the safest sex toys today.
The medical community has weighed in and determined that there are no health risks or side effects associated with using penis sleeves.
Penis sleeves are a safe, non-invasive, and non-pharmaceutical method of male enhancement as well as a solution for ED and premature ejaculation (PE).
There are a few risks associated with penis sleeves—but only if you don’t use them the right way. Remember these safety guidelines, and you won’t have to worry.
👉 Keep it clean: Properly cleaning your cock sleeve is the most important part of staying safe. Penis sleeves are often made from delicate and porous materials that can host mold and bacteria. If you neglect your sleeve’s cleanliness, it can become infected—and pass that infection on to you and your partner. Luckily, you can easily clean your sleeve with warm water and antibacterial soap. Clean it as soon as you’ve finished using it, and let it dry completely before putting it away.
👉Wear the right size: You should never feel any pain or intense discomfort when wearing a cock sleeve. If you do, it may mean you’re wearing the wrong size. As with any sex toy, if you feel pain, stop.
👉 Be gentle: Be gentle to your cock sleeve and your partner. Being too rough can hurt both of them. Cock sleeves made from skin-like materials are more delicate and can tear. And remember that your partner may not be used to such a big member.
👉 Get consent: Never use a penis sleeve on an unsuspecting partner. Just because they’ve consented to sex does not mean they’ve consented to using a sleeve. Surprising someone with a penis sleeve can lead to injury. If you’re not sure how to breach the subject, check out this article on how to introduce sex toys into your relationship.
